Scheduling an appointment
Scheduling an appointment is done via closest to you office at Walnut Creek or San Francisco location.
Our staff will send you a “Welcome Package” with required information and directions to our
offices.
Before you come in for your first appointment, you will be asked to complete Urogynecologic
diagnostic questionnaire and Medical history questionnaire as well as
Three-day urinary diary.
These are not only to make you visit more efficient but even more importantly allow you to
concentrate on your symptoms and reflect them accurately on provided forms, far before your visit.
Frequently patients find that signs and symptoms they for many years assumed being "normal"
consequences of childbearing and/or aging represent significant alterations of pelvic organs
functions. These questionnaires became very important diagnostic and educational tools for us
and our patients. Please, do not forget to bring them with you. Also obtain and bring all
associated previous medical information i.e. reports of previous operations, consultations
and applicable tests.
In order to expedite your visit and to offer the best guarantee that all pertinent elements will
be covered, we ask you to provide us with some information. Using enclosed form "Patient
Registration & Insurance Information", please, fill out demographic and insurance information as
listed in the forms. Bring all appropriate insurance information including insurance cards.
If you belong to HMO insurance, you must obtain a written authorization (not a referral) from
your Primary Care Physician or Ob/Gyn; according to HMO insurance policies services cannot be
provided without an authorization. If you have any questions regarding our billing procedures
and policies, please, see Financial Policy or contact our office manager. This will allow
processing your claim most efficiently.
Expect to spend about 1-1½ hours in direct consultation with our physician. For some patients
presented information might become overwhelming and it becomes vitally essential that you bring
with you to the office one close able relative, i.e., husband, daughter, son, sister,
daughter-in-law, granddaughter, etc. We strongly encourage family members, significant others
and/or friends to accompany a patient to this consultation to learn more about their problems
and prospective treatment. This will allow them to participate in decision making, future care
as well as planning ahead. It will also diminish phone calls to our staff for "clarifications".
We also would like to ask for your voluntary participation in our clinical research project The
nature of you participation is limited to answering questions related to the impact of your
symptoms on your quality of life and will take only a few minutes. Questionnaires are provided
as a part of a "Welcome Package". Once again you participation is voluntary and regardless of
your decision you care will not compromised. If you would decide not to participate in the study,
please, bring blank forms to our office at the time of your visit.
